There are several reasons he doesn't call you back. I mean if he doesn't wanna call you, there could be his reason. But we'll just pinpoint it to the two most important meanings to that;

1. He doesn't like you enough. He's just not that into you. He's busy. However, you want to phrase it. 

There's meaning in that absence. Move along. Next! Why are you waiting by the phone anyway? He should be waiting for you_ Quoted.

2. He was with something that came up. But even at that, you may say! Well, probably at the time he was about to call back, you called him. For this point, you got to know each other's schedules, what his busy day is like, and what yours as his lady is. 

Occupation is also something to consider, considering him being a Medical Doctor or the like, for example, picking your calls often or even calling often isn't going to be that easy.
Understanding becomes very vital.

Just watch out for the two, and observe what you are noticing in your relationship because you've got to ask yourself; "Is he busy or it's just a lame excuse? Because in the end, you're the one in the relationship and your emotional stability is nothing to jeopardise with!
